By the way, it has three (3) staff working on field and one (1) left in the office. However, they are small in numbers of employee but the company was very rich having the complete equipment for any events that they venture. Event Avenue was founder by two (2) siblings Jhoanna Jane and Nerisa S. Javier. The sisters live somewhere in Lagao, Generaal Santos City. The sisters were raise by a Christian Family. The Business could be affected having a Christian owner to establish a very trustworthy company to dwell by the customer. One thing one the Event Avenue they not very particular of the profit they could gain in the business. They believe honest and trustworthy could lead them more into success for just being appreciated by their customer through their hard work. This would be make a difference having a great virtue in them believing things you do right can make an output of righteous also. STATEMENT OF THE PROBLEM The study will be directed towards the end of the result which solves the following problems, impediments and difficulties encountered with the manual approach: Can the proposed system manage the record of the clients? Will the proposed system able to secure and keep the records of client transactions? How reliable and accurate was the proposed system compared to manual service of booking? Will the proposed system able to generate hard copy reports? OBJECTIVE OF THE STUDY The main point of study is to create a system which can give specific recommendations of the clients and also to have manageable and organized systematic reference of transactions table. RESEARCH PROCEDURE The following are the step by step procedure in conducting our research: First, the researchers discussed by sharing their own ideas to make title proposals. Ninth, if the system experiences an error the researchers must start debugging the program and start testing again until there were no more bugs in the system. RESPONDENTS OF THE STUDY On our study, they were four respondents that approved our study. First, the respondent whom is the person-in-charge in the operation of the company and so the person we’ve explained about the goal, objective and other concerns of the system studied, followed by the other three staff which are the people in-charge in the field operation like person in-charge in technical, production and designing which are the persons we asked through surveying  and interviewing them, introducing to them the proposed system. We start conducting our interviews and surveys the things about what are the things we need to do and the things which are they suggest to put on the system just to be effective. DESIGN IMPLEMENTATION This chapter talks about the design implementation of feasibility of the study, operational feasibility, technical feasibility, economic feasibility of the propose system. The benefit of this design implementation is to impart knowledge to all the users to aware the software and hardware capability implementations of the propose system. This system implementation could help the Events Avenue to have their transaction upgrade and manage orderly. FEASIBILITY STUDY The feasibility of the study is all about online booking process introducing it through the company Event Avenue bringing upgrade their transactional process manual into systematic approach to how it will do manages and organize their any transaction. OPERATIONAL FEASIBILITY The main objective of the study is to develop a system which can give a big benefit into the Events Avenue in terms of their services through making their transaction become systematic by keeping their data manage orderly and can be helpful to the clients and the same time to the employee of the company having a transaction online and keep the record safely without hassle to input it manually. The hardware and software were the main consideration for the functionalities of the system to avoid such difficulties and malfunction of the system. Upon after installing the various systems, we abruptly build the form of the study and the needed attachment for the implementation. This will be the basis of the proponent to determining succ essful implementation. And also, the proponent not just talks about the outcome of the study but also the possibility of encountered errors by implementing the study successfully. As the proponent trying our best to produce a better outcome for even though it  is not that quite easy. CONCLUSION In regards to all the data that gathered especially from the data surveys along with the questionnaires in the respondent Event Avenue – Pionner, General Santos City, therefore, we conclude that the Study can give the Administrator an easily operation and knowledgeable outcome of better manageable transactions, that the study can give the Event Avenue a better and convenient work regarding to how the operation going for newly approach of systematic process that the technology can give, that the study will give Event Avenue a profitable and beneficial process that certain IT industry can modify and upgrade. We as the proponent want that this online booking system become upgradable and well develop so many can benefit through this kind of system and so many will be satisfy by it works and lessen the hardship regarding the manual transaction with the clients. However, it should need the personal interaction with the clients. RECOMMENDATION As the proponent of the system, we intend to have a system which is beneficial to the users and helpful in terms of transacted records. To improve the quality and performance of the system and as well the security and the reliability of the system can give to the users. This propose system can be improve by developing its features and its transactional data recorded, creating more sequence regarding the design and transaction handle or catered by the system letting the user enjoy the capability and fullness of the system.

According to Lord Devlins formulation, a duty of care arose only when there existed a relationship Equivalent To Contract[1], between the claimant and the defendant, an application of the general conception of proximity, between the two parties. In the given scenario a special relationship between the parties was non-existent. Alberts reliance on Barrys advice was unjustifiable, as the loss suffered here was not attributable to the defendants negligent mis-statement; he had not voluntarily assumed responsibility towards the claimant. A duty of care would only arise if the defendant foresaw the claimants reasonable reliance on his statement. The case of HEADLEY BYRNE CO. LTD v HELLER AND PARTNERS LTD[2](HOUSE OF LORDS, 1964) applies to the given situation. Here the court held that if a professional person in the course of his business imparted advice, knowing that it was being relied upon, then he owed a duty of care to that person, to exercise reasonable care and skill, failing which, he would be liable in negligence. However, a disclaimer prevented any duty of care from arising. Since, the above is not applicable to Barry, he did not owe any duty of care to Albert. The advice I would give Mrs Smith is to impose a claim, as the loss suffered by her because of not claiming her insurance was attributable to the defendants negligent mis-statement. He had voluntarily assumed responsibility towards her and therefore it was his duty to find out about any changes in law that affected her position. He owed her a duty of care and was clearly in breach of that duty. It would be reasonable to sue him in the County Court in order to make good the loss or otherwise try for an out of court settlement to avoid legal costs. The damage to the melt was an economic loss while the loss of profit on that melt was consequent thereon but loss on profit caused by the power cut was not directly consequential upon any damage done and therefore a pure economic loss, and not claimable. Percy can therefore claim for the economic loss as well as for consequential losses thereon. However, he cannot recover the pure economic losses that were independent of the physical damage. POEMS Syndrome Symptoms and Treatment POEMS Syndrome: Paraproteinemic neuropathies, Organomegaly, Endocrinopathy, M-protein and Skin changes Abstract The POEMS syndrome, also known as Crow-Fukase syndrome, is a rare multi organ disorder characterized by polyneuropathy, organomegaly, endocrinopathy, M-protein spike and skin changes. Other associated features, such as sclerotic bone lesions, edema, ascites, hematological disorÂÂ ­ders and Castleman disease can also be present. We report a case of POEMS syndrome who presented with insidious onset, progressive sensorimotor polyneuropathy, pedal edema, ascites, hepatomegaly along with skin changes. X-ray pelvis showed osteosclerotic lesions. Thyroid function tests showed hypothyroidism. M-protein (IgG) monoclonal band was seen on immunoelectrophoresis. The patient was started on melphalan and corticosteroid combination therapy. We emphasis on the importance of recognizing a challenging diagnosis of a rare disease, which is shown to be treatment responsive. Introduction POEMS syndrome is a rare paraneoplastic disorder of plasma cell dyscrasias, which was first described in 1956 by Crow and then in 1968 by Fukase [1]. The name POEMS was given to it by Bardwick and co-workers in 1980 based on five salient features: polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy and skin changes [1]. It is more prevalent in men, with male to female ratio of 2.5:1. It usually manifests in 5th and 6th decades of life. Its inheritance is uncertain and its pathophysiology is still not well understood. Case Presentation A 40-year-old male presented with progressive weakness, tingling and numbness sensation in both lower limbs for two years. He had swelling of lower limbs, abdominal distention and dermatologic changes in form of discoloration and thickening of skin over the cheeks, nose, hands and feet for one year. He had also gave history of erectile dysfunction and loss of libido for six months. There was no history of syncope, bony pain or drug abuse. He had no previous history of hypertension, diabetes or tuberculosis. On physical examination, the patient had bilateral pedal edema [Figure 1.c] and abdominal distention [Figure 2.a]. Skin was thickened and hyperpigmented over the face, fingers of the hands and shin (Figure.1a,b,c). Bilateral gynecomastia and testicular atrophy were present. Abdominal examination showed hepatomegaly and ascites. Higher mental functions and speech were normal. Fundus examination showed papilledema on both side and rest of the cranial nerves examination were normal. Motor power in upper limbs was normal and in lower limbs showed predominant distal weakness [Medical Research Council (MRC) 4/5 at hip joint and 4-/5 at ankle joint). Deep tendon reflexes in upper limb were diminished (+1) including biceps, triceps and supinator and absent in lower limbs. There was 30% loss in pain, touch and temperature sense in both lower limbs below knees. Posterior column sensations (joint position and vibration sense) were also impaired in lower limbs below the anterior superior iliac s pine. Romberg sign was positive. Hemogram, liver and renal function tests, muscle enzymes (creatine phosphokinase), serum ferritin and vitamin B12 level were normal. Serum total protein was 7.1 gm/dL, albumin 3 gm/dL, globulin 4.1 gm/dl, and A:G ratio 1:1.3. Fasting and postprandial blood sugar level were normal. Thyroid function test showed raised TSH level (16.62 ÃŽ ¼/ml). Luteinizing hormone (LH) and testosterone levels were 15 IU/L (1.8-8.6 IU/L) and 111 ng/L (300-1,000 ng/dL), respectively. Antinuclear antibody (ANA), Rheumatoid factor (RF), serum human immunodeficiency virus (HIV) ELISA test, hepatitis B and C markers were negative. Ultrasonography of abdomen showed hepatomegaly (16 cm), moderate ascites and enlargement of multiple lymph nodes along the iliac vessels. Fine needle aspiration cytology of mesenteric lymph node was inconclusive. Ascitic fluid examination revealed exudative nature (SAAG Discussion POEMS syndrome is a rare, multiple system disorder, characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal or M-protein band and skin changes. Any three of the five features may be present to establish diagnosis [2]. However, some authors have proposed clinical criteria for diagnosis in which includes two major criteria, which can be either presence of polyneuropathy or plasma cell proliferative disorder. Minor criteria include sclerotic bone lesions, organomegaly, edema, endocrinopathy, papilledema or skin changes [2]. Polyneuropathy is a predominant feature of POEMS syndrome and is found in >90% of the cases. It is usually a sensorimotor, axonal and demyelinating type polyneuropathy [3]. As in our patient, both axonal and demyelinating polyneuropathy are seen on electrodiagnostic studies. The mechanism of neuropathy is not known but the recent evidence of the presence of anti-neural antibodies points to an immunological mechanism [4]. Endocrinopathies occur with a frequency of 60%-80% and the most common are gonadal failure (70%) and glucose intolerance/diabetes mellitus (50%). Hypo or hyperthyroidism, hyperprolactinemia and adrenal insufficiency have also been reported. The mechanism of endocrinopathy is also not obvious; however, involvement of direct acting antibodies against hypothalamo-hypophyseal-axis and endocrine end organs has been hypothesized [5]. In our patient, impotence, loss of libido and testicular atrophy and hypothyroidism on ancillary laboratory investigation were present. Increased levels of vascular endothelial growth factor (VEGF) are found in POEMS syndrome. VEGF increases microvascular permeability, thereby inducing edema, ascites and pleural effusions as were present in our patient [6,7]. However, measurement of VEGF level in ascites was not available in our patient. Papilledema may be seen in approximately 37% of patients and is not associated with the increase in intracranial pressure. The real cause of papilledema is not still known. Hepatomegaly may be seen in up to 50% of patients with splenomegaly and lymphadenopathy occurring less often. The hyperpigmentation over the face, legs and hands was also observed in our case. The skin changes usually observed in POEMS syndrome are hyperpigmentation, lichenification, hypertrichosis, sclerodermoid changes and glomeruloid hemangiomas. Skin biopsy may show inflammation, fibrosis, or nonspecific changes. Monoclonal protein is detected in more than 90% of patients and may become positive in the follow- up of patients who have no monoclonal gammopathy initially [8]. Nearly all cases reported in the literature show lambda positivity as in our patient. It may be rarely found in urine and cerebrospinal fluid (CSF). The natural course of POEMS syndrome is chronic, with a reported median survival for a decade (8-13.8ys). The morbidity depends on the extent and number of systems involved. The cause of POEMS syndrome is still unknown. It is tempting to incriminate the presence of lambda light chains in the pathogenesis because of their unexpected frequency (more than 95% of patients), but histopathologic review of affected organs and nerves does not support that it is a form of deposition disorder. Increased levels of cytokines IL-1ß, TNF-Ã ¡ and IL-6, more specifically VEGF, appear to play a pathogenic role in the disorder [9,10]. In view of the constellation of a mixed polyneuropathy, monoclonal gammopathy, osteosclerotic myeloma, extravascular volume overload, bilateral papilledema, skin changes and endocrinopathies, our patient was diagnosed as POEMS syndrome. However, other close differential diagnosis like tuberculosis and hemochromatosis were ruled out with appropriate investigations. Patient was treated with combination of alkylating agent melphalan and corticosteroid. To conclude, when a patient present with unexplained sensorimotor polyneuropathy, signs of extravascular volume overload and evidence of other system involvement, a high index of suspicion should be kept for a diagnosis of POEMS syndrome, to avoid missing this rare syndrome, which is amenable to treatment. Duty of Care in Health and Social Care

Duty of care is a legal obligation for each individual in the health and social care setting that requires them to adhere to a standard of reasonable care. Ensuring they don’t put their service users or themselves in any danger.In the workplace there are policies and procedures, agreed standards, codes of practice and other legislation a care worker should follow.In a care workers job role you would be responsible for making sure the service users’ needs are met to the best of your ability making sure the service user does not come to any harm and also making sure they are involved in their care plan. Promoting services user’s choice and rights to the best of your ability. You would be responsible for assessing possible risks.You must remain professional throughout you role making sure you are ad-hearing to confidentiality, keeping up to date and accurate records of the care you have or are providing to service users.If you are not sure about any part of your wor k you or have concerns then you must speak to the manager straight away to make sure that no mistakes are made.Duty of care is central to all that you do at work, it is not something extra.Q 1.2 Explain how duty of care contributes to the safeguarding or protection of individuals?A 1.2Duty of care contributes to the safeguarding or protection of individuals by keeping individuals safe whether it is illness, abuse, harm or injury. We can do this by involving families, health care professionals and other external agencies into the individuals care plan.Duty of care is a legal requirement and there are policies, procedures, code of conduct and legislation around safeguarding and protecting your service users. Following these guidelines is showing that we are providing the best care possible.If you are doing activities with service user you should always do risk assessments making sure that the service user is aware of any risks also.Q 2.1 Describe potential conflicts or dilemmas that m ay arise between the duty of care and an individual’s rights?A 2.1Potential conflicts or dilemma’s between the duty of care and individual rights is about enabling service users rights to do what they want to do but making them aware of risks and the harm to others. You cannot stop the service user from making a choice. We all take risks in everyday life for example walking across the road.When there are concerns about and service user’s capacity to understand risks and consequences of their actions there is an â€Å"Incapacity Test† to assess the capacity. If the service user does not have the capacity then it is down to the people caring for the service user to make decisions. It is easy to assume that a service user does not have the capacity to make decisions based on their disabilities.A potential conflict or dilemma that may arise is if a service user wishes to smoke. The service user has the right to smoke and for an area to be set up for them to s moke but you can also make them aware of the risks involved or  harm to others that can be caused.Another conflict or dilemma which may arise is if a mental health patient is refusing to take medication. The patient has the right to refuse to take medication but as a care worker your duty of care is to try and explain the risks and harm that can be caused by the patient not taking their medication. You can seek help from other professionals i.e. Psychologist, GP, Mental health nurse, Family members (as they may listen to the family more than professionals because they may feel that professionals are trying to harm them.).When dealing with dilemmas or conflicts it helps to seek advice and guidance from other people such as colleagues, manager, service users family members, and other professionals connected with the individual.Q 2.2 Describe how to manage risks associated with conflicts or dilemmas between an individual’s rights and duty of care?A 2.2You can carry out a risk assessment that involves the service user so they fully understand the risk/s they are taking. If the service user still wishes to take the risk then you have to try and make it as safe as possible for them to do so, by doing this you are meeting your obligation to provide duty of care. Update care plans and paperwork to show that you have explained the risks to the service user.Q 2.3 Explain where to get additional support and advice about conflicts and dilemmas?A 2.3There are many different ways to receive extra support to help with dilemmas and conflicts. You can ask colleagues as they might have had to deal with a similar situation or may have other ways to help. Line manager as they are  more experienced, other professionals working with your service user i.e. doctor, social services, schools or colleges, counselling service who may know how to . You are never alone in making a decision where there are conflicts or dilemmas.Q 3.1 Describe how to respond to complaints?A 3.1Exp lain to the service user the procedure for making a complaint.Listen to what the individual is saying without interruption and assure the client that you are interested in their concern.Reassure the person that you are willing to do something about their complaint and are glad that they have brought it to your attention.Never make excuses, get angry or blame other staff.Provide the service user with information and advice on how you are going to deal with the complaint and in what time scales. Making written details of this also.Report the complaint to your line manager and reflect on the complaint to improve your professional development.Q 3.2 Explain the main points of agreed procedures for handling complaints?A 3.2There are two ways to make a complaint verbal and non-verbal. If a complaint is made verbally you should usually deal with this complaint straight away unless you are unable to do so, at this point you would ask you line manager or another colleague for help to deal wit h the complaint.If someone makes a non-verbal complaint there is usually a procedure in place to respond to the complaint within a certain timescale usually 2-3 days. Usually the manager will respond to these type of complaint. However it is important to find out what went wrong and how, this is usually done in a meeting with the complainant and the investigating manager. The next phase would be about putting the complaint right and making sure that they do not occur again. When complaints are handled in this way it is referred to as local Resolution.If the complainant is not satisfied with how the complaint has been resolved they can complain to the Local Government Ombudsman to be investigated further. Complainants can also complain to the care quality commission.As a care worker you will be given a policy and procedure on how to handle complaints this is usually in your code of practice.
